Brook Psychological, PLLC understands that making the decision to take care of your health and wellness is an investment of time and money. Dr. Slife is committed to working together to help you see and feel the benefits of this investment.

 

Treatment begins with an Initial Evaluation appointment which is typically scheduled for 55 minutes. Ongoing therapy appointments typically take place weekly for 55 minutes. The frequency of appointments will be agreed upon according to client needs and may change throughout treatment.

 

Appointment Pricing:

 

The full price for services is as follows:

Initial Evaluation, 55 minute appointment $200 

45 minute therapy appointment: $150 

55 minute therapy appointment: $180 (typical session)

Payment is due at the time services are rendered. Methods of payment accepted include cash or major credit card. This includes flexible saving account (FSA) cards and health savings account (HSA) cards. If you choose, your card information can be kept securely on file for convenient processing.

Insurance: Medical/health insurance may cover some or all of the costs associated with your treatment. Dr. Slife is in process of getting on several insurance panels and currently accepts Medicare.

Out of Network/Private Pay: Dr. Slife accepts private pay and can provide a "superbill" for you to submit to your health insurance company for reimbursement.
